REGISTRATION AND PAPERWORK


You will need the following paperwork for registering your soccer player with TFA. All forms can be downloaded on our website.

U5-U10 TFAL:

  • TFAL Contact Form.


U9/10 Academy Teams, U11/12/13/14 Youth Teams and U15/16/17/18 High School Teams:

  • Please print clearly and fill out in pen!
  • TFA Contact Information Form - This form needs to be submitted at tryouts. If the form is filled out at tryouts, you will not need to submit another one.
  • TFA Consent and Waiver form - Every player has to submit a new form, including existing TFA players.
  • OYSAN Player Membership Form (for both SL and MRL) - Every player has to submit a new form, including existing TFA players.
  • Medical Release Form - We cannot accept this form unless it has been notarized. Every player has to submit a new form, including existing TFA players.
  • Copy of a Birth Certificate - New players only; existing TFA players do not need to submit a new one.
  • Passport Size Color Picture - Please print your name and team on the back of the picture. Every player has to submit a picture, including existing TFA players.
  • TFA Uniform Form (if applicable) - Uniforms can be sized at The Soccer Spot, Maumee.
  • Minimum Deposit ($150) + Uniform Fees - All forms and deposits will guarantee your position.


* Out of state residents (Michigan). You will be required to pay an additional fee (around $25) to request permission to play in Ohio North. This requires additional paperwork. This is mandatory for all out of state residents.
We schedule a paperwork meeting at The Soccer Center, Maumee after tryouts (date and time will be confirmed by email). This allows you to try on uniforms and drop off your paperwork. A notary will be present for the medical release form.
